Series used to document action taken on workers’ compensation claims of city employees.  Series includes correspondence, medical records, payment records, copy of vendor payment log, claimant payment records, and claim forms.

8702-02   Employer at Injury Program (EAIP) Records

Series documents program to return injured employees to work by reimbursing employer from the Reemployment Assistance Reserve.  Series includes doctor’s approval, description of task, invoices for expenses incurred by employing agency, bid information, correspondence, memorandum, copy of reimbursement check from state and how dispersed.
