6208-01   Material Disposal Log Records

Records are created to document where collected material is being disposed and is used to track types of material that may be recyclable. Record contains material type, amount, where material was disposed, employee disposing of material and the date material disposed.

Retention: 4 years

6208-02   Material Chemical Test Result Record

Series is used to check and analyze material for PCB's, pesticides, hydrocarbons, petroleum, and other substances as identified in the EPA test guidelines. Records are reports compiled by outside testing agencies containing the breakdown of recycled material samples sent out for independent testing. Report includes name of testing lab, lab report, and certification of test results.

6208-03   Recycling Sales Record

Series is used to track amount of recycled material sold by the Bureau of Maintenance. Record includes original service request, copy of invoice, amount of material sold, billing amount, delivery information, how paid, customer information (i.e. name, address, phone, signature) and employees signature.

6208-04   Recycling Mailing Lists

Series is used to notify customers of the availability of recycled material. Record contains name, address, and phone number of past customers. Record also includes type of material purchased and date purchased. Record may also include copies of letters sent to the customer.
