You may submit your application by mail or in person.
- The deadline to submit an application is by 9 AM two weeks prior to the Committee’s regularly scheduled meeting. The Committee meets on the first Thursday of the month.
- A complete application, including all supplemental information and the application fee must be received by the deadline for an application to be considered by the Committee.
- Applications will be screened for completeness. Submittal of inaccurate or incomplete forms may cause a delay in the review of the application.
