Emergency Form, Medication, Allergies & ADA Accomodations

Emergency Form:

  • Please complete the camp waiver-emergency form and bring it with you the first day of camp.  Your child will not be able to participate in camp without the Medical/Emergency Form completed. There is a different waiver-emergency form for each camp season. Summer, Winter & Spring.
  • Everyone must fill out the NEW form for summer, winter & spring, as we update all of our records for camp before every season. It is important to us      that we ALWAYS have the most up-to-date information on your child.
  • Swimming: Children that are marked on the emergency form as not needing to wear a life jacket will be asked to complete a swim check with lifeguard      supervision on the first day in the pool. The child will need to swim the length of the lap pool. Lifeguards look for kids to be able to swim the whole length without getting tired or requiring support from the wall.  Children who do not take the swim check, or do not successfully pass the  swim check, are required to wear a life jacket. A child may re-take the swim check. A swim check need only be completed once for the season.  Preschool camps do not swim.
  • Field Trips: EPCC offers four separate destination field trips every summer. Field trips will change from year to year. All participants will need to complete a separate Camp Field Trip Waiver before the trip. Click on Summer Camps for more details.

Medicine:

  • If your child requires medicine to be administered or carried with them while they are at camp, such as an EpiPen or inhaler, you will also need to complete a Medication Waiver and a Medical Administration Form. Please complete these forms and bring them with you on the first day of camp, or plan for additional time at check-in to fill them out.
  • All medication must be in its original container and bring only the amount needed for one day at a time.
  • If you have any questions about medicine and camp, please call 503-823-0206 and speak with the coordinator in charge of the camp.

Allergies:

  • If your child has allergies, please notify the front desk at the time of registration. The Customer Service Representative will include this information on the alerts in your account.
  • You should also contact the coordinator in charge of the program at least 5 days in advance so that accommodations can be made where possible for planned activities.

ADA Accommodation:

  • Within the Americans with Disability Act (ADA) of 1990, Portland Parks & Recreation provides accommodations to individuals with disabilities/special needs, so that they may receive an equal opportunity to fully participate in programs offered city-wide.
  • When registering, let the Customer Service Representative know that you will need an accommodation.   You will also need to contact Inclusion Services at 503-823-4328 to request accommodation a minimum of 10 business days (Monday-Friday) before the start of the program.   • EPCC works closely with families needing accommodation within EPCC camp programs. If you are planning to register for one week or every week of camp with EPCC and your child needs ADA assistance, please contact the EPCC Inclusion Coordinator, Gabe Barbee at 503-823-0206 or gabe.barbee@portlandoregon.gov . We hire for Inclusion Assistants in early May, the sooner we start a conversation, the better prepared we will be.
