Once you have reserved your trampoline party or event by paying the security/damage deposit, you are ready for the next step - paperwork! Whoopee!
Before the staff at Montavilla Community Center can open the trampoline for your groups use, we MUST have both the:
Trampoline Use Agreement: The usage agreement form must be read, understood & signed by one adult (over 18 years old) who will be on-site during the event as the person responsible for the event/party. Please click HERE to download the usage agreement OR request a hard copy of this form from the front office. This form must be signed & on-file at Montavilla prior to the beginning of your event.
Participant Waiver: Staff on-site will collect waivers as guests arrive for your event. Those with completed waivers will have access to the trampoline, those without can still come-in to run & play in the gymnasium with their friends but will not be allowed to jump on the trampoline. It is the renters responsibility to print & distribute either the Party Invitation with Waiver OR just the waiver itself. Simply click on the links provided below to download either item. Please contact the staff at Montavilla Community Center if you do not have access to a printer. We will provide 20 invitations or waivers for your use.
