The HOME and LIHTC (Low-income Housing Tax Credit) Programs share many forms for verifying incomes of potential tenants and recertifying incomes of existing tenants. However, the programs do have some differences so please use the proper forms for the program(s) administered in your project.

The asterisk (*) indicates forms that are for the HOME Program only; otherwise the form is for use with both HOME and LIHTC Programs. Visit the Oregon Housing and Community Services (OHCS) website where the LIHTC Compliance Manual and other forms and information may be found.
