Annual Reporting Tools

To access and use the Asset Management and Streamline portal for annual reporting, you will need a user-defined password, assigned by the Asset Management team. Contact Louise K. Glur Lauman at Louise.Glur@portlandoregon.gov or 503-823-0468 to get access to the portal. After logging in, your "unique list of properties" will be available. Each project will identify all required reporting documents for that specific reporting cycle.


Affirmative Fair Housing Marketing Plan (AFHMP Form) (Word)

Download and save the AFMHP form to your computer. The form can be filled out by tabbing through each field. Required reporting is every five years. If updating is required, the portal will identify this requirement.

Asset Management Plan (Template)

This plan should be completed and provided at loan funding for all projects. The plan provides strategies and policies for the project. The plan also details key personnel, financial, property management and resident service plans, if applicable.

Corrective Action Plan (Template)

This template details the process and items to start the process for consideration for loan restructure and/or regulatory modification. After completion, this template -- along with the requested information -- is submitted to Asset Management.

HOME Compliance

This Asset Management program monitors project and Borrower compliance related to HOME funding received from the City of Portland
