  • Taxi
  • TNC (Do not fill out paperwork- please apply directly with company)

Driver Accident Report

Drivers are required to report accidents while working.

Driver Certification/Re-Certification Form

Driver Certification/Re-Certification TO BE FILLED OUT, SIGNED, AND SUBMITTED BY AUTHORIZED COMPANY REPRESENTATIVE ONLY. Required for new drivers and annually for renewing drivers. 

COMPANY OWNERS: Company (instead of the City of Portland) can conduct background checks on employees if preferred- see link for accredited companies: NAPBS (National Association of Professional Background Screeners).

Driver Background Check Authorization Form and Instructions

Background Check Authorization Form (to be filled out by DRIVER after Company Representative submits Certification/Re-Certification form. Includes disqualifying factors) and “What to Expect”.

Study Guide NEW Taxi and LPT Driver Test/Training

For NEW applicants. Test does not need to be taken at PFHT office- can be taken online at links below.  Register with name, email, last 4 of your Social Security Number, and Driver License Number. We will receive the results and note that you have passed the test.  Taxi, TNC and LPT drivers are required by Portland City Code to pass the test within 30 days from the date driver permit is approved. Test can be taken once every 72 hours and must be passed with a score of 80% or better. Study Guide can be referenced during test. Test can also be taken in the PFHT office Monday-Friday 10am-3pm.

  • NEMT (Non-Emergency Medical Transportation)
  • Pedal Powered/Non-Motorized vehicles

PLEASE NOTE: Until portion of City Code pertaining to NEMT, Pedicab, Pedal Powered and Horse Drawn Carriages is revised and approved by Council, applicants must continue to apply directly with the City of Portland and follow the rules and regulations in Code previous to changes made January 1, 2016 (see TRN 14.25 - Interim Administrative Rule).

Out of State Driving History Record Request Form

Form with fees for applicants requesting the City of Portland retrieve records. Applicants can also request record(s) directly from former state(s) of residence and submit to the City of Portland with completed driver application/renewal. See application for requirements.
