7210-01   Citizens Concern Records

Series is created to track specific concerns, complaints and comments about the maintenance and operation of the parks. Series is used to respond to citizens and to document their concerns. Records include copies of correspondence, follow-up memos, investigations, and other records used to explore and answer citizens comments. Series includes reoccurring concerns and tickler records created to track those issues. Series is generally maintained by District Supervisor.

Retention: Retain until problem resolved or no longer needed.

7210-02   Park Bureau Resource Guide (Parks Finder)

Series is a resource guide developed to answer questions and provide general information about the parks and districts. Series is used to gather information about the park districts that may be found in other places in a single easy to use guide. The resource guide contains information about the various parks and park districts. Information includes parks in the district, district foreman, personnel lists, responsibilities, maps, information about the parks in the system, and addresses, phone numbers and fax numbers fir the individual parks, districts and employees.

7210-03   OSHA Injury Records

Series is created to meet federal requirements to summarize injury and occupational illnesses. Records include name, case number, date, description of injury/illness and other related information. Examples of records include OSHA 200 log, OSHA 200 report, databases created to track injuries and other similar types of records.

Note: Some records contained in these files may be exempt from disclosure under ORS 192.501 through 192.502. Consult your agency's legal counsel for assistance before releasing information.

7210-04   Site Inspection Records

Series is created to document inspections of parks and facilities regarding safety issues. Series is used to identify potential problems regarding safety issues. Records include description of problem, date of report, solution (if any), and other related info. Records may include correspondence, reports, memos and photos. Includes inspections to check compliance with consumer products safety requirements.

Retention: 3 years after superseded
