6212-01 Bureau Identification Cards

Series is used as a back up to the identification issued to Bureau employees. Record contains photograph of employee, employee name, classification, identification number and personnel information.

6212-02 Personnel Issue Investigation Records

Records are used to document personnel issues that explore specific concerns. Records may be exploratory in nature. Records include correspondence, memos and consultations with the union, employees, Bureau of Personnel, and Bureau of Maintenance managers. Records may include documents used to resolve issues.

Note: Some records contained in these files may be exempt from disclosure under ORS 192.501 through 192.502. Consult your agency's legal counsel for assistance before releasing information.
