Please feel free to print this form.  It is a 1/2 page form set up for back-to-back printing of two per page.  For consistency, we ask that you print it on bright yellow paper, preferably card stock.

Once printed, the page can be cut in half to give you two copies of the Refrigerator Card.  Please print the information in the areas provided and mount the card on the front of your refrigerator.

Updating the information as changes occur will help your emergency responders to help you more accurately and efficiently.
