An “exemplar practice”

Congratulations to Central City Concern’s Old Town Clinic, which has been named an “exemplar practice” by the Robert Wood Johnson Foundation.

The Robert Wood Johnson Foundation (RWJF) is the largest philanthropy foundation devoted to health and healthcare in the United States.  RWJF has teamed up with the Group Health Research Institute to create a national program designed to identify creative practices that make primary care more accessible. The project goes by LEAP, Learning from Effective Ambulatory Practices.  

The clinic will play host in mid-February as the LEAP team will do a three-day visit to assess how the clinic makes staffing arrangements to make primary care more accessible and effective for patients. Best practices will be put into training, and technical assistance materials to create a better resource for patient care-driven facilities.

The Old Town Clinic is a federally qualified health center, and is operated by Central City Concern (CCC). CCC's Executive director Ed Blackburn and his staff are part of Health Share of Oregon, the largest Coordinated Care Organization in Oregon.

Three cheers for the Old Town Clinic for its fantastic work, and for the care it provides to the residents of Portland!
