Health Form

Environmental Education uses ePACT, an online health information system, to collect all health and emergency information for campers.

Prior to your first day of camp, you will receive an invitation from Portland Parks & Recreation to enter your child's information into ePACT  (if you haven't seen it by the Wednesday before camp beings, check your spam!). Click the link in the invite, create an account, and then take the time needed to fill out all contact, authorized pick-up, healthy, allergy, and emergency information for your child(ren). As a final step, please make sure to  click "submit", which shares your child's information with our program so we have access to it in an emergency.

Filling out the profile takes 15-25 minutes, but once completed it is available to be used for any week of PP&R camp (regardless of program or location). Compared to our old paper forms, ePACT is more thorough, more convenient, and more secure than a paper form - plus it is HIPPA-compliant. We rely heavily on ePACT each day of camp - our on-site and off-site coordinators have access at all times to your child's profile on our City of Portland cell phones and iPads.

Children who have not completed a profile in ePACT by the first day of camp will be required to fill out a temporary paper form to attend camp and then complete the online information in ePACT before day two of the program.

The Environmental Education Coordinator for this program is Chrissy Larson. Feel free to contact Chrissy or the Environmental Education office with questions, feedback or concerns.
