Day Camp Forms Check-List

Completed forms provide us with necessary information as we strive to keep your children safe and happy during their day at camp. Please be sure to complete and submit any and all forms needed for your child based on the check list below. Forms can be printed online, are available for pick up at the office at Montavilla Community Center, or can be requested to be sent via mail or fax by calling us at 503-823-4101. If you choose to fill the necessary forms out when you drop your child off at camp, please allow 5-10 minutes to complete the paperwork so you are not in a rush and are able to supply full and accurate information. 

 

Participant Emergency Form

  • Please complete the Participant Emergency Form and bring it with you the first day of camp. Your child will not be able to participate in camp without the Medical/Emergency Form completed.
  • Everyone must fill out the NEW form for each season of camp (Winter Break, Spring Break, & Summer), as it is important to us that we ALWAYS have the most up-to-date information on your child.
  • Field Trips: Our Daily Adventures camps may travel in school buses to local area destinations on field trips during Summer Day-camps. Please check the confirmation information on your receipt and the field trip list in the Summer catalog for the location of your child's field trip.

Medication Administration Forms

  • If your child requires medicine to be administered while they are at camp you will also need to complete a Medication Waiver and a Medical Administration Form. Please complete these forms and bring them with you on the first day of camp, or plan for additional time at check-in to fill them out.
  • All medication must be in its original container and bring only the amount needed for one day at a time.
  • If you have any questions about medicine and camp, please call 503-823-4101 and speak with the coordinator in charge of the camp.

ADA Accommodation

  • Within the Americans with Disability Act (ADA) of 1990, Portland Parks & Recreation provides accommodations to individuals with disabilities/special needs, so that they may receive an equal opportunity to fully participate in programs offered city-wide.
  • When registering, let the Customer Service Representative know that you will need an accommodation. You will also need to contact Inclusion Services at 503-823-4333 to request accommodation a minimum of 10 business days (Monday-Friday) before the start of the program.
